Walgreens Mail In Rebate

This form must be printed and mailed in with original store receipt. 

 

To be eligible for $10 off rebate, you must: 

1. Buy any Diabetes Doctor™ product between 12/08/2021 and 3/31/2022. Online purchases are not valid for rebate. Each customer is allowed only 2 rebate submissions (up to $20 total rebate value). 

2. Purchase in-store at any Walgreens or CVS Pharmacy. Online purchases are not valid for rebate.

3. Mail this form along with the original store cash register receipt, dated between 12/08/2021 and 3/31/2022 with your product purchases and price circled.
